Upon discharge from the hospital, a nurse will come to your home to assess your individual needs. A plan will be put in place to assist you in your recovery to return to your previous state of health if not better! Our nurses conduct a thorough interview and assessment of your needs and wants upon admission into OakMed.

Every OakMed patient will be assigned a dedicated case manager and be provided with direct contact information. No 800 numbers. No call centers to try and get questions answered.

Includes in-home care services for patients after an auto accident or work accident. Care at home is provided by professionals who are licensed, insured and bonded. We take additional steps to guarantee safety and quality care within the home setting with rigorous hiring standards, nurse supervision, 24 hour on call and clinical support and ongoing training and education of all staff.
